| Applying to be an Undergraduate Student |

| First-Time Freshmen |
| |
| Students who would like to enter UL Lafayette as first-time
freshmen are required to submit the following to UL Lafayette Office of
Admissions, PO Box 41210, Lafayette, LA 70504 to be considered for admission: |

- An Application for Admission. This form can be obtained from
Enrollment Services Phone: (337) 482-6553 or toll free (800) 752-6553.

- Application Fee. The application must be returned with a
$25 non-refundable application fee - check or money order, NO
CASH.
- Official ACT or SAT Scores. If the student does not have
scores on file at UL Lafayette, the student must request official
ACT scores, from his/her high school, or provide a copy of his/her
original scores to the Admissions Office. UL Lafayette's ACT Test
Code is 1612. SAT test scores will be converted to an ACT equivalent.
UL Lafayette's SAT Test Code is 6672.
If the student has never taken the ACT/SAT exam, he/she may
be eligible to take ACT residual exam administered on the UL
Lafayette campus.
- Immunization Form. It is mandatory that all new students
show proof of immunization for measles, mumps, rubella, tetanus-diphtheria-pertussis
(whooping cough), poliomyelitis, hemophilus influenzae type B, and meningococcal
meningitis. Please complete and submit the Immunization
Form to
the Office of Admissions.
- Register with the Selective Service by visiting www.sss.gov
- High School Transcript. First-time freshmen applicants may
submit a sixth or seventh semester transcript for initial admission
and scholarship consideration. A final transcript showing graduation
date and grade point average must be received prior to enrollment.
Unaccredited Louisiana High School graduates, GED recipients, and
Home Study graduates must submit official proof of graduation or
bring original diploma to the Office of Admissions.
If you attend
a high school in Louisiana, you may wish to check with your guidance
counselor if your school participates in the Louisiana Department
of Education Student Transcript System (STS). If your school
participates in this program, we will be able to access your high
school record
electronically. |

| 25 Plus Adult Admission |
| |
| By definition, a 25 Plus Adult Student must be a degree-seeking
first time freshman 25
years of age or older. Adults must have graduated from high school or
have received their GED. Adults admitted under 25 Plus do not meet the
published admission
requirements for the current semester; however, they will be offered
Guaranteed
Admission. This admission status entitles 25 Plus Adults to the same
services available
to other degree-seeking students. |

| To be considered for admission as a transfer student, submit

- College Transcripts. Transfer students must have each collegiate
institution attended mail an official transcript directly to the
Office of Admissions. If you are currently attending a university,
we
encourage a partial record to determine initial eligibility. Submit
a final official
transcript upon completion of any
classes currently attending. Please send the Request
for Transcript of Credits Form (PDF) to each institution when
requesting transcripts to be sent to UL Lafayette. Official transcripts
are mailed directly from the institution to UL Lafayette. Hand
carried copies/transcripts are not considered official.

| By definition, a re-entry student is a student who has not
attended another collegiate institution since last attending UL Lafayette.
Applicants for re-entry to UL Lafayette must submit the following items

LA 70504 after one or more regular semester(s) of nonattendance at

| To be considered for admission as an international student,

- Application Fee. The application must be returned with a $30 non-refundable
application fee - check or money order, NO CASH .
- Completed Financial Statement. All International applicants
must submit a completed Confidential
Financial Information Form (PDF) if they wish to be admitted to
the University of Louisiana at Lafayette. This form provides evidence
of the applicant's ability to pay all required educational and living
costs. The Confidential Financial Information Form must be on file
with the Admissions Office at UL Lafayette before an I-20 or IAP-66
can be issued. This is not required for United States citizens or
applicants with permanent resident status.
- TOEFL Scores. Original results of the Test of English as
a Foreign Language (TOEFL). A minimum score of 525 paper based
(195 computer based) is needed for admission.
- Immunization Records. It is mandatory that all new students
show proof of immunization for measles, mumps, rubella, tetanus-diphtheria-pertussis
(whooping cough), poliomyelitis, hemophilus influenzae type B, and meningococcal
meningitis. Please complete and submit the Immunization
Form for the application process
and bring all your medical documents to the Office of Admissions
by the deadline date (below)
for the semester in which you are seeking admission:

| International Student Deadlines: |

| Priority will be given to all applicants with complete requirements
received by the stipulated deadlines. The deadlines will apply to all international
applicants whether they are currently residing inside or outside of the United
States. |

- First-Time Freshmen (Have not attended a post-secondary institution)
- Submit official or certified true copies of all post secondary
academic records, transcripts, notes and/or mark sheets from foreign
institutions and U.S. schools, if applicable. A course program syllabus
should accompany the records from foreign institution(s).
- Transfer Students (Have attended a post-secondary institution)
- Submit official or certified true copies of all post-secondary
academic records, transcripts, notes and/or mark sheets from foreign
institutions and U.S. schools, if applicable. A course program syllabus

- NOTE: The appropriate letter of admission and visa form will
be issued only after all credentials have been received, and the
applicant has been admitted to the University.

| Student and Exchange Visitor Information System Fee |
| Important New Information for International Students |
| |
| Effective 1 September 2004, international students (F-1 and
J-1 visa applicants) applying for admission to an American university are
required to pay a SEVIS I-901 fee of $100.00 US to the United States Department
of Homeland Security. |
